The Personnel Action Form (PAF) is a form used to request all personnel actions (offer letters for hires, FTE changes, title changes, termination/resignation, supplements, reappointments etc). Please send a completed PAF to paf.request@jax.ufl.edu. You will receive a confirmation email when it’s been received, along with information on what additional documents are needed to complete the action requested.
