Purpose: To recognize UF College of Medicine – Jacksonville Faculty dedicated to Patient Experience by achieving a five star physician rating calculated from annual transparency scores

Deadline: Awards calculated with scores from January 1- December 31. Award ceremony celebrated in the Spring

Frequency: Yearly award

Commitment to Excellence in Patient Experience Award

Purpose: To recognize UF College of Medicine – Jacksonville faculty with representation from Primary Care, Surgical Specialties, and Specialty Care who have demonstrated a dedication to patient experience by being identified as the Physician who has most improved their overall scores from the provider section of the patient experience survey.

Sponsor: Dean’s Office/ Office of Patient Experience

Award: Desk Ornament

Deadline: Awards calculated with scores from January 1- December 31 as compared to the previous January 1-December 31. Award ceremony celebrated in the spring

Frequency: Yearly award

Compliance Advocate of the Year Award

Purpose: In recognition and appreciation of the commitment to ethical business practices and continued support of compliance efforts at the University of Florida College of Medicine – Jacksonville and the University of Florida Jacksonville Physicians, Inc.

Sponsor: Office of Physician Billing Compliance

Awards Amount: Plaque

Deadline: Based on Compliance Committee member recommendation. Announced each spring during scheduled quarterly Compliance Committee meeting.

Physician Impact Award

Purpose: To recognize UF College of Medicine – Jacksonville faculty with representation from Primary Care, Surgical Specialties, and Specialty Care identified by the Patient Advocacy Council Patient Experience subcommittee committed to patient experience on our downtown campus demonstrated by a combination of patient experience scores, and a dedication to excellence in patient care by promoting, maintaining, and achieving health equity for all patients.

UF W. Martin Smith Interdisciplinary Patient Safety Awards Program

Purpose: This program provides start-up support for faculty and staff to design and implement projects focused on reducing the likelihood of adverse events or claims and/or patient safety and clinical process improvements. Projects selected for funding may also address national initiatives such as the Partnership for Patients, CMS improvement programs, and IHI and NQF initiatives. All faculty, employees, staff, residents, fellow, and students of the University of Florida Health colleges and at UF Health Shands in Gainesville and Jacksonville are invited to submit applications.

Sponsor: Co-sponsored by the UF College of Medicine Continuing Medical Education and the UF Self-Insurance Program (SIP).

Awards Amount: Most Smith Awards projects will be in the amount of $25,000 or less over the life of the project.

Deadline: 3 month open submission period from January 1 to March 31

Frequency: One award cycle per year – 1 winner for Gainesville and 1 winner for Jacksonville
