The University of Florida Jacksonville Pain Research Participation Recruitment Registry

Are you interested in participating in the UF Health Jacksonville Pain Registry? We are creating a registry to build and maintain a list of participants who wish to be contacted about future pain-related research studies.

If you choose to join the registry, we will ask you for:

• Your name
• Your contact information
• Basic personal information: birth date, gender, ethnicity, race, marital status, language spoken & education level

The research team may contact you periodically to ask for your voluntary enrollment in studies you qualify for. They will also reach out once per year to help verify and update your contact information in the registry.
