The Center for Convening Transformative Care What is the Center for Convening Transformative Care? The Center for Convening Transformative Care (CCTC) is the hub for linking population health initiatives, care pathway innovations, and solutions to impact the social factors to establish more equitable health care delivery platforms for the UF Health Jacksonville patients and the communities in which they reside. Established in 2024, the CCTC facilitates the programs, initiatives, and projects taking place within the various UF Health entities. Collaborative efforts within UF College of Medicine – Jacksonville, College of Nursing, College of Pharmacy, and UF Health Jacksonville hospitals create the framework to maximize these impacts for patients, educational efforts, research and overall community outcomes. By uniting these collectives and amplifying communication, the CCTC will establish additional mutually beneficial external collaborations. Within a framework of data analysis, program management, lean methodology, and academic rigor, the center’s goal is to create a coherent quality roadmap supporting the UF Health vision of compassion, excellence, respect and stewardship enhancing community value.
