Upon completion of the presentation, participants will be able to:

1. Describe health disparities among patients with chronic diseases in the Jacksonville community among residents of the urban core.
2. Explain the five components of the collective impact model.
3. Recognize the importance of addressing unmet social needs to improve clinical outcomes and cite examples of effective community health interventions
