At the conclusion of this session, the participant should be able to:
1. Describe the drivers of health for youth and young adults with special health care needs.
2. Compare and contrast the concepts of healthcare transition and health care transfer.
3. Identify strategies to implement healthcare transition services into the outpatient and inpatient setting.
